SUBMIT AN ​APPLICATION

​CONTACT US

​The first step is to contact us in order to have all the information necessary for your entry into the program and to receive the forms. You can take the opportunity to ask us all your questions. You can contact us by email or by phone. For more information, see the Contact section.

​FILL OUT THE FORMS

​Once you receive the forms, you will need to print and complete them. You will also need to have a section completed by a health professional, such as a family doctor, psychologist, psychiatrist or a medical specialist related to your condition. Once the forms are completed, you can scan them and send them to us by email so that we can analyze your request.

​ASSESSMENT OF YOUR FILE

​Once we receive your documents, we will evaluate your application as soon as possible. We will call you as soon as the analysis is complete to discuss with you and inform you of our decision.

​HAVE YOUR DOG EVALUATED

​Your dog will need to be evaluated to determine if they are suitable to become a service dog. For more information on the evaluation, see the ​​Have Your Dog Evaluated section. If you wish, you can have your dog evaluated before completing the forms. If you do not have a dog, we can help you select a suitable dog from a trusted shelter or breeder.

ADMISSION

​If your application is approved and your dog has passed the initial evaluation, you will then be admitted to the program. Upon admission to the program, you will be required to pay the admission fee and provide us with a photo of your dog so it can appear on their identification card. Upon admission to the program, your dog will receive a vest and an identification card identifying them as a Service Dog in Training.

TRAINING PLAN

The dog trainer will contact you to establish with you a training plan adjusted according to your needs and abilities. This plan will serve as a guide for the trainer and you throughout the training.

TRAINING

​You and your dog will receive personalized training as outlined in the training plan. The frequency of training will vary depending on your and your dog's needs and abilities. For more information, see the Process section.
